To request to update this information do the following: send an email or fax request to 407-830-0398. Please include your full name, last 4 digits of your social and email address.
Be advised only the above information will be changed/updated. Requests to update any other information will not be processed. Any other changes to your application can be made once your application is pulled to receive assistance. You will receive a written response within 90 days.
