This form is not to be used to check the status of wait lists or your status on a wait list. Please call 407-706-3261. If you recently completed an application, please visit our home page and read the news updates regarding the recent open waiting list. 

This form is for two reasons:

• A participant who needs a hearing
• A person who has been removed from the waiting list and would like a review of the decision
